It simply adds the games gpd to your profile, making it look like you’ve played the game. If you’re a Diamond member you can then add achievements for that game with Horizon.

That’s not possible unless the game or DLC is DRM free. The only full games I know of that are DRM free are Alien Breed 1, 2 & 3, 2 Serious Sam HD games, Doritos Crash Course and Harms way.
