I have a modded Xbox 360 S (4GB). It was running flawlessly for about a year, but since few recent months I’m unable to play the latest titles, including Watch Dogs, MGS: Ground Zeroes, Fifa 15 etc. All the old games are running well. When I try to play latest titles it shows an update to install when I click “OK” it shows “The disc is unreadable”. Same error for all the latest games. Help me out please.

Actually, I don’t have any working knowledge of modding. I sent it to an electric shop got in done.
mmm… I think not, cause it’s been really long since an update prompt appeared when I connected to Xbox Live but the update couldn’t be installed, I guess because of being modded.
You have to manually update it. Download XeBuild 16747, you don’t want the newest 16767 unless you plan on going online. Download flash360 on your pc and put it on a usb on your xbox and you will have a .bin file called flashdump or something. Open that in XeBuild and set the dash to 16747 then hit rebuild. It will create a new file called updflash.bin, whack that in the same location as your flashdump on your usb and run flash360 on the console again, choosing update instead of dump. Once it’s completed you should be on newer dash. Then all you do is install the avatar/kinect dash files.

I’ve downloaded everything that’s needed and all set to go. But since it’s my first time doing so, I need afew basic info… I haven’t modded the console myself so I’ve no idea whether it’s Jtagged or RGH. Can you tell me a way to figure it out, since writing the dump file is different in both.
Open Dashlaunch and it will tell you in the bottom right what you have.
Nah if you just use Flash360 to dump it, then create a new image using xebuild then either xell or flash360 to write it back.

Got it… It’s Glitch2, Which means its RGH… So, for RGH Xell is used to write the nand file, right?
